Greater Upper Valley Integrated Services Team

GUVIST provides space for willing partners to learn an integration process using Collective Impact principles and incubate integration solutions, and provides technical assistance to cultivate service delivery networks, facilitate regional needs into action and help partners move beyond collaboration to integration of leadership, decision-making, operations, and responsibility.  Integration of service delivery improves services and support for community members to be efficient, effective, responsive and appropriate to the desires of those in need, where and when they are needed. All health care & social service organizations serving any population in Orange, Windsor, Sullivan and southern Grafton counties are welcome to join our monthly virtual meeting.

GUVIST is comprised of diverse health care & social service organizations who recognize the need for integration to improve service delivery for the community.

Where

In the bi-state Greater Upper Valley of New Hampshire and Vermont. We aim to serve portions of Orange, Windsor, Sullivan and southern Grafton counties.

What

GUVIST facilitates service integration to improve the experience of people who receive services from multiple organizations by helping service providers operate interdependently by design.
